Have you checked the logs that are mentioned in the upgrade tool? There is no way others can guide you if you are not providing them. I suggest you to check the logs and see what the issue is.

Please check the logs of CHECK phase, It seems pre requisites notes are not applied as per SAP upgrade guide. You will find such details in Check phase logs.

Turns out it was an SAP problem, they had to create a new ACP file for SAP_BS_FND. Once that was in place, the upgrade continued.
